How IHSS Authorized Hours Are Calculated in California
The number of hours you can receive (as a recipient) or get paid for (as a provider) comes from a formal assessment conducted by a county social worker. Understanding how that assessment works helps you advocate for the care you actually need.
The IHSS Assessment Process
When a new recipient applies for IHSS — or when an existing recipient's needs change — a county social worker visits the home to conduct an assessment. This visit typically includes:
How Are Hours Assigned?
Each IHSS service category has a time standard — how many minutes the state estimates a typical task takes. For example:
- Bathing: X minutes per week based on functional limitation level
- Meal preparation: Y minutes per meal × meals per day
- Protective supervision: can be authorized up to 283 hours/month for severe cognitive impairment
All service times are added together to produce total authorized hours per month.
What Factors Affect Your Total Hours?
- Severity of your disability or medical condition
- Level of cognitive impairment
- Whether you live alone (living alone often results in more hours)
- Whether you have other sources of support (family members in the home)
- Documentation quality — strong doctor letters increase authorized hours
Can You Get More Hours If Your Needs Increase?
Yes. You can request a reassessment at any time if your condition has changed. Contact your county IHSS social worker and explain that your needs have increased. Bring updated medical documentation.
If you are denied an increase, you have the right to appeal within 90 days of the Notice of Action. Request a State Hearing through CDSS.
