How to Increase Your IHSS Authorized Hours in California

If your current IHSS authorized hours are not enough to cover your care needs, you have options. The process involves requesting a reassessment, building a strong case with documentation, and — if necessary — appealing through the state hearing process.

When Should You Request a Reassessment?

You can request a reassessment when:

  • Your medical condition has gotten worse
  • You have a new diagnosis or injury
  • Your daily functioning has declined
  • Your current provider is spending more time than authorized to meet your needs
  • Your living situation changed in a way that affects your care needs

Step 1: Contact Your County Social Worker

Call your county IHSS office and ask to speak to your assigned social worker. Tell them you are requesting a reassessment because your needs have changed. If your social worker has changed, the office will connect you with the current one.

Follow up your verbal request with a written letter or email stating the same — this creates a paper trail.

Step 2: Prepare Strong Medical Documentation

The most powerful tool in a reassessment is updated medical documentation. Ask your doctor to write a letter that:

  • Describes your specific functional limitations in detail
  • Explains which daily tasks you cannot perform independently
  • States the time and assistance required for those tasks
  • Notes any recent changes in your condition
Specialist letters (from neurologists, physical therapists, psychiatrists, or occupational therapists) carry particular weight.

Step 3: Be Specific During the Assessment Visit

During the home visit, describe your limitations clearly and specifically. Do not downplay how much help you need. Use examples:

  • "It takes me 45 minutes to bathe because I cannot lift my left arm above my waist"
  • "I need reminders every 2 hours because I forget to eat and take medications"
  • "Without supervision, I have left the stove on multiple times"

Step 4: If Denied, Request a State Hearing

If the reassessment results in a denial or fewer hours than you need, you can appeal. Request a State Hearing within 90 days of receiving your Notice of Action:
