How Long Does the IHSS Process Take? A Complete Timeline

2026-05-30

The IHSS process typically takes 30–45 days from the time you submit your application to the time a provider receives their first paycheck. However, depending on your county, your specific situation, and whether all paperwork is completed correctly the first time, the timeline can range from as few as 20 days to several months. Here's a complete breakdown of every step.

Step 1: Submit the Application (Day 1)

The IHSS process begins when a recipient (or someone acting on their behalf) submits an application to the county IHSS office. You can apply:

  • In person at your county social services office
  • By phone — your county IHSS office will mail you an application
  • Online in some counties via the California Benefits portal
What you need at application: The recipient's full name, date of birth, Social Security number, address, and information about their disability or medical condition that requires care.

Timeline: Same day — the application is submitted and a case number is assigned.

Step 2: Social Worker In-Home Assessment (Days 7–21)

After your application is received, a county social worker will contact you to schedule an in-home visit. During this visit, the social worker evaluates:

  • The recipient's functional limitations (ability to bathe, dress, cook, etc.)
  • The home environment
  • The level of care needed in each IHSS service category
  • Whether the recipient is Medi-Cal eligible
What to expect: The assessment typically takes 1–2 hours. The social worker will ask detailed questions about what the recipient can and cannot do independently. Be specific and accurate — hours are assigned based on documented need. Timeline: Most counties schedule the assessment within 2 weeks of receiving the application. Some high-volume counties (Los Angeles, Alameda) may take 3–4 weeks.

Step 3: Notice of Action — Hours Authorized (Days 21–35)

After the assessment, the county issues a Notice of Action (NOA) that specifies how many IHSS hours per month the recipient has been approved for and which services are authorized.

Important: You have 90 days to appeal if you believe the hours authorized are too low. Keep a copy of the NOA. Timeline: NOAs are typically issued within 10–14 days of the in-home assessment.

Step 4: Provider Enrollment (Days 25–40)

Once hours are approved, the provider — whether a family member, friend, or hired caregiver — must complete the IHSS provider enrollment process:

  • Submit the SOC 426 (Provider Enrollment Form) to the county
  • Complete LiveScan fingerprinting (background check)
  • Complete the IHSS Provider Orientation (in person or online)
  • Sign the IHSS Workweek Agreement
  • Timeline: Provider enrollment typically takes 7–14 days after all documents are submitted. Background check results from LiveScan generally return within 3–5 business days.

Step 5: Timesheets and First Payment (Days 35–50)

Once the provider is enrolled and the recipient's hours are authorized, timesheets can be submitted. IHSS uses an Electronic Services Portal (ESP) for timesheet submission — both the recipient and provider must have ESP accounts.

    • Timesheets are submitted every two weeks (pay period)
    • First paychecks are issued on the Friday following the close of the pay period
Timeline: Most providers receive their first payment 35–50 days after the original application was submitted, assuming no complications.

    What Causes Delays?

The IHSS process can take significantly longer when:

    • Medi-Cal is not yet active — IHSS requires Medi-Cal eligibility. If a Medi-Cal application is pending, the IHSS process cannot complete until Medi-Cal is approved.
    • Missing documents — Incomplete provider enrollment forms or missing LiveScan results pause the process.
    • High case volume — Counties like Los Angeles have high application volumes that can push timelines to 60–90 days.
    • Appeals — If the recipient or provider disputes the authorized hours and files an appeal, the process pauses pending the hearing outcome.

How to Speed Up the IHSS Process

    • Submit complete applications with all required documents the first time
    • Confirm your county received your application and ask for a case number immediately
    • Complete LiveScan fingerprinting as early as possible — it's often the longest-running step
    • Set up your Electronic Services Portal account before your first timesheet is due
    • Follow up weekly with your county IHSS office if you haven't received your NOA within 3 weeks

    FAQ

Can IHSS be expedited? In some circumstances, yes. If the recipient faces an urgent health or safety situation, contact your county IHSS office and ask about expedited processing. Documentation from a doctor or hospital supporting the urgency can help. Does IHSS pay retroactively? No. IHSS pays from the date of provider enrollment approval, not from the date of application. This is why completing provider enrollment quickly is important. Can I be my own provider under IHSS? No. The recipient and provider must be two different people. However, immediate family members — including adult children, spouses in some cases, and siblings — can serve as providers.
