New IHSS providers in San Bernardino and Riverside counties are reporting enrollment processing waits of 8 to 12 weeks — more than double the state’s 30-working-day target — leaving care recipients with approved services but no authorized caregiver able to bill for hours worked. Advocates say the backlog is worsening as IHSS demand grows faster than counties can staff enrollment processing units.
How Provider Enrollment Is Supposed to Work
Before an IHSS provider can be paid for care hours, they must complete enrollment with both the county IHSS program and the California Department of Social Services (CDSS). The standard enrollment process includes:
- Live scan fingerprinting — a Department of Justice criminal background check
- Identity verification — government-issued ID and Social Security verification
- County IHSS office review — registration in the county’s provider database
- CDSS provider enrollment — registration in the statewide Medi-Cal provider system
Under normal processing conditions, CDSS targets completing enrollment within 30 working days of receiving a complete application. Providers who enroll as a new IHSS caregiver for a family member often experience faster processing because the recipient is already active in the system.
What’s Happening in San Bernardino and Riverside
In San Bernardino County — which serves approximately 45,000 IHSS recipients through roughly 35,000 providers — a combination of higher-than-expected new recipient approvals and understaffing in the county’s IHSS enrollment unit has pushed processing times well beyond the 30-day target.
Multiple providers who contacted Unified Savers described submitting complete applications in March and April 2026 and receiving enrollment confirmation in late May and early June — waits of 10 to 12 weeks. During that period, some were caring for their recipient unpaid, expecting to be reimbursed retroactively once enrollment cleared.
Retroactive pay for the waiting period is not automatic. IHSS providers cannot bill for hours worked before their enrollment effective date. In most cases, the enrollment effective date is the date CDSS completes processing — not the application submission date. Hours worked during the backlog period may be uncollectable unless the provider can document hardship and request a retroactive effective date, which requires county IHSS supervisor approval on a case-by-case basis.
Riverside County, which serves approximately 32,000 recipients, shows a similar pattern. Riverside County IHSS staff confirmed to Unified Savers that processing times have extended to 8–10 weeks due to a combination of enrollment staff vacancies and increased application volume following the county’s expansion of IHSS eligibility for adults aged 18–21 with developmental disabilities in January 2026.
The Impact on Recipients
The practical consequence of enrollment delays falls hardest on recipients. A care recipient whose approved IHSS provider cannot bill for hours faces one of three bad options:
- Go without care — not feasible for recipients with high-need or protective supervision hours
- Use an unenrolled provider informally — legal risk for both recipient and provider; the provider cannot be paid
- Use a different enrolled provider — often not possible when the new provider is a family member or the only available worker in a rural area
San Bernardino County covers a large geographic area including the High Desert and mountain communities where provider availability is already limited. A recipient in Victorville or Yucca Valley with a willing family member in enrollment limbo has very few alternative care options.
What CDSS Is Doing to Address the Backlog
California’s Department of Social Services acknowledges enrollment processing delays as a systemic issue and has implemented several measures aimed at reducing backlogs:
Expedited enrollment for urgent care situations: Recipients facing a care emergency (hospitalization discharge, death or incapacity of existing provider) can request expedited 10-business-day enrollment processing. Requests must be submitted by the recipient’s IHSS social worker and accompanied by documentation of the urgency.
Digital application improvements: CDSS launched an updated online IHSS provider enrollment portal in March 2026 that is intended to reduce incomplete applications — the most common cause of delays — by prompting applicants through required document uploads before submission.
Temporary staffing augmentation: CDSS has requested Budget Act funding to add 45 full-time enrollment processing positions statewide. The positions are pending final budget approval expected in mid-July 2026.
What New Providers Should Do to Protect Themselves
If you are enrolled or waiting for enrollment in San Bernardino, Riverside, or any county with known processing delays:
- Document your application submission date — keep copies of all documents submitted and any confirmation numbers or emails from the county or CDSS
- Do not begin working for pay before enrollment is confirmed — working before your enrollment effective date creates payment problems that are very difficult to resolve after the fact
- Ask explicitly about retroactive effective dates — if you submitted a complete application and the delay is entirely on the county/CDSS side, request that the county IHSS supervisor consider an earlier effective date
- Contact SEIU 2015 if the delay exceeds 30 working days — the union has established processes for escalating enrollment disputes with county IHSS offices
- Request a status check every two weeks — call the county IHSS enrollment unit directly; passive waiting extends delays because incomplete or misrouted applications can sit unresolved for weeks without follow-up
