- Legally blind: Defined as visual acuity of 20/200 or less in the better eye with the best correction, or a visual field limitation of 20 degrees or less
- Disabled: A medically determinable physical or mental impairment expected to last at least 12 months or result in death, and that prevents substantial gainful activity; or a condition that limits the ability to perform two or more activities of daily living without assistance
People receiving SSI/SSP (Supplemental Security Income/State Supplementary Payment) automatically satisfy both the income and disability criteria for IHSS because SSI enrollment automatically confers Medi-Cal eligibility in California.
3. California Residency and Functional Need
The applicant must be a current California resident — there is no minimum length of residency required. Immigration status requirements are addressed separately below.
Beyond residency, the applicant must demonstrate a need for assistance with activities of daily living (ADLs) that, without IHSS support, would require placement in a nursing facility, hospital, or other institutional care setting. The key question the county social worker evaluates is: Can this person safely remain at home without paid in-home support?
Qualifying functional needs that IHSS can cover include:
- Personal care: bathing, grooming, oral hygiene, dressing, bladder and bowel care
- Mobility assistance: transferring, ambulation, positioning in bed or wheelchair
- Meal preparation and cleanup
- Domestic services: housecleaning, laundry, essential shopping
- Accompaniment to medical appointments (travel time included)
- Paramedical services: administration of medications, wound care, range-of-motion exercises
- Protective supervision for recipients with cognitive impairments who cannot be left alone safely
Immigration Status and IHSS Eligibility
California’s IHSS program includes expanded coverage compared to federal Medi-Cal rules. As of 2026:
- U.S. citizens and lawful permanent residents with qualifying immigration status have full IHSS eligibility if they meet Medi-Cal requirements
- Undocumented California residents aged 50 or older can access full-scope Medi-Cal under SB 104 (expanded to all ages in 2024), which in turn makes them IHSS-eligible if they meet functional and income criteria
- DACA recipients and certain other immigration statuses qualify for Medi-Cal and IHSS under California’s expanded eligibility rules

The IHSS Assessment Process

Who Does Not Qualify for IHSS
